Hat Sizing

Place a piece of string or tape measure around your head about 1/8" above your ear, across the mid-forehead, completely circling your head. Hold the tape firmly, but not too tightly. Basically you need to measure your head exactly where the hat will sit. If your measurement falls between sizes, choose the next largest size.

The chart that @Giles posted is for US sizes. In the UK they're 1/8 of a size smaller. For example, my head has a 60 cm circumference, and I wear a 7 3/8 English hat and a 7 1/2 US.
